Multi-Agency Request Form: Reporting concerns

Non-Emergency: If you believe that a child is at risk of significant harm but they are not in immediate danger you must complete a Multi Agency Request Form (MARF) as soon as possible and within a maximum of 24 hours.

Please send your completed MARF via secure e-mail to: access_team@sandwellchildrenstrust.org

Emergency: If a child is at immediate risk of significant harm please call 999 in the first instance for police and/or ambulance services and then contact Sandwell Children’s Trust on 0121 569 3100. You will then need to complete a MARF without delay.

If you have safeguarding or child protection concerns in relation to children please consult the SCSP Multi Agency Thresholds Guidance; Continuum of help and support and if necessary make contact with Sandwell Children’s Trust.

If you believe that Multi Agency Early Help services will be able to assist the family please complete and Early Help Assessment, using EHM, (training is available) to help identify the most appropriate Lead Professional and a plan of support. If the family have complex needs and you are unable to identify a Lead Professional amongst partners already involved submit the Early Help Assessment to the MASH For consideration for Strengthening Families Service support.

Paper copies of the Early Help Assessment can be requested by vie this email address Ecaf_Queries@sandwell.gov.uk  (Please note that Sandwell professionals will be asked to prove that they are booked on EHM training in order for a paper referral to be accepted.)
