The tools below are available to practitioners to support the identification of neglect. The tools can be selected and utilised as appropriate.  They do not replace an Early Help Assessment or a Social Work Assessment, but can be used to complement existing assessment tools.  Application of professional judgement is always required and any information elicited from use of the tools needs to be triangulated with information from other sources.

Neglect Screening Tool

The screening tool is designed to support the identification of neglect. It asks practitioners to rate any areas of concern using the Signs of Safety rating scale.  The tool is a series of prompts, not an exhaustive list, and other areas of concern can be added to it if necessary. The screening tool does not replace the Early Help Assessment and can be used alongside it.  Whilst it is not a requirement, when making a referral to the Family Front Door due to neglect concerns you may be asked if the screening tool has been completed.

Home Conditions Assessment /Home Conditions (progress tracker)

This tool asks a series of questions about different aspects of the home environment. It can be completed with parents and/or children to develop a shared understanding of any concerns about the home conditions and to agree a plan about what action needs to be taken to address them.  Those completing the assessment are asked to rate each area using the Signs of Safety rating scale.  The tracker can then be used to record any changes in each area to evaluate progress.

A Day in My Life

In order to understand a child's lived experience it is necessary to have a picture of what it feels like to be him/her living within their family.
